Solving for x in a quadtratic equation

The problem is solve for x.
x^2+12x+4=0

Here is what I did:
(-12±?(?12?^2-4(1)(4)))/(2(1))
(-12±?(144-16))/2
(-12±?128)/2
(-12±?(36*5))/2
(-12±?36*?5)/2
(-12±6?5)/2

I have my answer as:
-6+3?5, -6-3?5
Is this accurate? If not, please show me what I did wrong…

(-12±?36*?5)/2
(-12±6?5)/2

Woops, I mixed two of my problems:) Let me see if I can correct:

Here is what I did:
(-12±?(?12?^2-4(1)(4)))/(2(1))
(-12±?(144-16))/2
(-12±?128)/2
(-12±?(64*2))/2
(-12±?64*?2)/2
(-12±8?2)/2

I have my answer as:
-6+4?2, -6-4?2
Is this accurate?
