I'm trying to understand the issue, so that I may offer useful suggestions.
What is the difficulty, when the book is on the bed? Are pages trying to turn or move, while you're using both hands to steady the camera? That happens to me, sometimes. I'll position on top of the book an object heavy enough to hold the page in place. (Sometimes, I need to take two pictures.)
Is the issue something else?
