It is impossible to "help" without knowing what you do know and can do with problems like these. This looks like a test on which you simply left most of them blank and gave answers with little or no connection to the questions on others.
For example, on e problem asks you to find a matrix that will give a "projection onto the plane x= 5". Do you know what that means? What would the point (x, y, z) be mapped to under that projection? Do you know why they want a "4 by 4" matrix for a three dimensional problem?
