factor by grouping if possible

12x^3+10x^2-30x-25

These just come with practice. Pay close attention to "-" signs.

12x^3+10x^2-30x-25
= (2x^2)(6x + 5) - 5(6x + 5)
= (2x^2 - 5)(6x + 5)

After factoring a polynomial completely, how do you tell if the polynomial is prime?

If you can factor a polynomial - that polynomial is NOT a prime.
