Equation to express a Medical Condition/Problem

Hi,
I am trying to create a formula to express a complex medical problem.
Not being versed in advanced math I am looking for help with the correct symbols to express my intent and the correct structure.
This is what I have done so far

Let me write it out in words
Degree Hearing Impairment equals a function of (degree Hearing Loss +function of the time duration of the Hearing loss + function of cognitive ability +function of memory ability + function of sozial activity level + function of education + function of vocabulary skills)

appriciate any advice and help.

How about expressing a variable over a period of time?

Maybe the variable V = 3t^2. That is V is a function of time=t. Is that what you mean?

You can still have a function of V. Maybe f(V) = 3V^2+5 = 3(3t^2)^2 +5= 27t^4 + 5
